The geography of the course consists almost entirely of backcountry gravel roads, with less than a mile of pavement in total. The route winds through one of the last remaining large-scale tallgrass prairie ecosystems in North America. A unique aspect of the Heartland course is its "open range" status; runners frequently encounter free-roaming cattle. Participants are instructed to remain calm and walk through herds, as the animals are generally curious but gentle. In early May, Kansas weather is notoriously volatile; while average temperatures range between 10°C (50°F) and 24°C (75°F), the exposed nature of the Flint Hills means that wind and sudden spring storms can significantly alter race conditions.
Logistically, the race is a strictly "no-pacer" and "no-dog" event due to the safety concerns involving grazing livestock. Packet pickup is conducted exclusively on race morning at the Cassoday Community Center from 4:30 AM to 5:30 AM. Runners have access to drop bag services at all manned aid stations, but crew access is limited to specific points (Lapland and Teterville) to prevent traffic congestion on the narrow gravel roads. For those pursuing the "Heartland Double," completion of the spring 50-mile or 50K distance combined with any Heartland distance in October earns a special commemorative award.
